Below are real NHRA practice MCQs from our Q&A bank. Each comes with an answer and explanation — just like the full bank.

Q1. A patient with congestive heart failure is prescribed furosemide. Which electrolyte imbalance should the nurse monitor for?

    1. Hypernatremia
    2. HypokalemiaCorrect
    3. Hypercalcemia
    4. Hypomagnesemia is rare

Explanation: Furosemide is a loop diuretic that causes potassium loss; nurses must monitor for hypokalemia (low potassium).

Q2. Which is the FIRST action when a patient develops anaphylaxis after IV antibiotic administration?

    1. Notify the physician
    2. Stop the infusionCorrect
    3. Administer diphenhydramine
    4. Place patient supine

Explanation: Stopping the offending agent (IV antibiotic) is the first nursing action, then assess airway and notify the physician.

Q3. The normal range for adult serum creatinine is:

    1. 0.1–0.4 mg/dL
    2. 0.6–1.2 mg/dLCorrect
    3. 1.5–2.5 mg/dL
    4. 3.0–4.0 mg/dL

Explanation: Normal serum creatinine in adults is 0.6–1.2 mg/dL. Elevated values indicate impaired renal function.

Q4. When teaching a diabetic patient about insulin injection sites, the nurse should recommend:

    1. Using the same site daily
    2. Rotating sites within the same anatomical regionCorrect
    3. Avoiding the abdomen
    4. Injecting only into muscle

Explanation: Site rotation within one anatomical area (e.g. abdomen) keeps absorption consistent and prevents lipodystrophy.

Q5. A nurse caring for a postpartum patient notes a boggy uterus. The priority nursing action is to:

    1. Notify the physician
    2. Massage the fundusCorrect
    3. Administer oxytocin
    4. Encourage ambulation

Explanation: Fundal massage is the first intervention for uterine atony to promote contraction and reduce hemorrhage risk.

NHRA Q&A Bank — Question Format & Coverage

The NHRA exam is a single-best-answer computer-based test delivered by Prometric under the authority of the National Health Regulatory Authority (NHRA), Kingdom of Bahrain, which does not publish the question count or seat time. Every question in the Phoenix RN NHRA Q&A bank uses the same format: a short clinical stem, four options and one best answer, with a written explanation that references the underlying rationale rather than repeating the answer choice.

The 6,000+ questions in the bank are tagged to the NHRA syllabus in the same proportions Prometric uses on the live paper. That means when you practise, your mix looks like this:

    • Medical-Surgical Nursing — 32% of the bank, covering Cardiac & vascular disorders, Respiratory & COPD, Renal & fluid-electrolyte and more.
    • Fundamentals of Nursing — 14% of the bank, covering Infection control & PPE, Vital signs & pain, IV therapy & wound care and more.
    • Pharmacology — 13% of the bank, covering Drug calculations, High-alert medications, Antibiotics & anticoagulants and more.
    • Maternal & Newborn Health — 10% of the bank, covering Antepartum & intrapartum care, Postpartum haemorrhage, Neonatal resuscitation and more.
    • Paediatric Nursing — 10% of the bank, covering Growth & development, Paediatric emergencies, Immunisation schedule and more.
    • Mental Health Nursing — 8% of the bank, covering Anxiety & mood disorders, Substance abuse, Therapeutic communication and more.
    • Community & Public Health — 8% of the bank, covering Bahrain MoH programmes, Chronic disease prevention, Health promotion and more.
    • Leadership, Ethics & Legal — 5% of the bank, covering Delegation & prioritisation, Patient rights, NHRA scope of practice and more.

    Most-Recalled NHRA Question Themes

Phoenix RN debriefs NHRA candidates after every attempt and feeds the recall back into the Q&A bank. The themes that show up most often on recent NHRA papers are grouped below — expect two or three MCQs from each in a typical sitting.

    Medical-Surgical

    • ECG rhythm identification
    • Diabetic ketoacidosis management
    • Chest tube care
    • Stroke FAST assessment

    Pharmacology

    • Warfarin food interactions
    • Heparin antidote (protamine)
    • Digoxin toxicity signs
    • Insulin peak times

    Priority / Delegation

    • ABC-based prioritisation
    • RN vs LPN delegation
    • First action in anaphylaxis

How to Drill the NHRA Q&A Bank

    1. Start topic-wise, not random. Drill medical-surgical first (it is 32% of the paper), then pharmacology, then the remaining domains in weight order.
    2. Aim for 50 questions a day for six weeks. That covers the entire bank twice — the second pass is where retention actually forms.
    3. Read the explanation for every question, including the ones you got right. On the NHRA paper, distractors are often "almost right" — knowing why they are wrong is what earns the mark.
    4. Bookmark every question you get wrong. Two days before your date, review only the bookmarks. Candidates who do this consistently score 10–15 percentage points higher than candidates who repeat the whole bank.
    5. Once you sustain 75%+ topic-wise, switch to full-length timed mocks to build the 3-hour stamina a full paper demands.
